Because of my obsession with symmetry, I'd want one set of twins with a boy and a girl, and a second set of a boy and a girl.

In that case I'd want them to be fraternal, otherwise I might have manly looking daughters or girly looking boys

Or for identical twins (which I'd prefer, sounds like fun) have one set be boys and the other be girls.

but overall I want my chil'ren to have at least one other same-gendered sibling, and for all of them to be of a similar age

Yes, the ears are excellent. I wouldn't worry too much about the eyelid. The touch-up of the nose should really top it off quite nicely though. If you really had some time though, I'd suggest putting the American flag behind him.
